Who Can Refer

We accept referrals from:

  • Local authority services

  • Social work teams

  • Commissioning teams

  • Placing authorities

Who We Can Support

We support individuals who:

  • Require personal care alongside independence development

  • Are preparing for adult life and independence

  • May be care leavers or transitioning from residential care

  • Can safely share accommodation with others

  • Are motivated to work toward independence

  • Have support needs we can safely and effectively meet

Who We Can't Support

We may not be suitable for young adults who:

  • Require nursing care or specialist medical support

  • Need secure accommodation

  • Require 24-hour intensive staffing beyond our model

  • Have needs that exceed our staff competency or registration scope

  • Present risks that cannot be safely managed in our setting

    Our Admission Process

    1. Referral and Information Sharing
    • We review comprehensive referral information from placing authorities

    • Initial compatibility and capacity assessment

    • Preliminary discussion about the young adult's needs

    2. Assessment and Matching
    • Meeting with the young adult and their professionals

    • Individual needs assessment including personal care requirements

    • Risk assessment and compatibility with current residents

    • Opportunity for the young adult to visit and meet the team

    • Assessment of whether we can safely and effectively meet their needs

    3. Placement Planning
    • Personal care needs assessment

    • Individual support plan development

    • Agreement of goals and outcomes

    • Transition and settling-in period agreed

    • Clear communication with all stakeholders

    4. Transition and Settling In
    • Gradual transition where appropriate

    • Clear welcome and induction for the young adult

    • Initial settling-in period with enhanced support

    • Regular review in early weeks

    5. Ongoing Partnership
    • Regular reviews with placing authorities

    • Close liaison with social workers and key professionals

    • Family involvement where appropriate and safe

    • Continuous assessment and adjustment of support

    What We Need to Make a Referral

    To enable us to assess suitability, we require:

    Comprehensive referral information

    Recent risk assessments

    Care plan or pathway plan

    Health information including personal care needs

    Education or employment status

    Relevant background and history

    Current support arrangements

    Funding and placement authority details
